All forms should printed and turned in on registration day.
#1: BSA Youth Application (New Scouts Only)
Application for scouts joining the BSA. The form may be filled out digitally or by hand. Please print and turn in the physical form. (Only page #3 is required.)
Note: The first "Date of Birth" field is for the scout's birthday.
#2: Contact Information - (New Families and Current Families with Address/E-mail Changes)
Register your contact information with Pack 122 so that we are able to communicate to you and share important documents about upcoming events.

#3: Annual Health & Medical Forms - Scout, Family, & Siblings
The BSA requires health and medical forms for all scouts and family members participating in Scouting activities. For Pack 122, only parts A & B are required. Forms must be updated and turned in annually.
Remember to include a copy of your health insurance card.
To make it easier to submit forms in the future we recommend that you save copies of the health form. fill them out digitally, and save them to your computer for future editing/printing.

#4: BSA Drug & Alcohol Policy Form
Our pack requires all families to agree to the BSA "Scouter Code of Conduct" to keep all Scouting event drug and alcohol free.
#5: Code of Conduct
All scouts must agree to the Pack 122 Code of Conduct. Please go over with your scout and ensure that they understand the expectations when participating in Pack 122 events.
#6: BSA & Pack Fees
$210 New Scouts / $185 Returning Scouts
Plus $65 for the Pack Support Fee (New Scouts $90, includes Class B Shirt)
